Care Coordinator

This role will see you responsible for ensuring the delivery of high-quality care & support services that are person-centred, committed to achieving client’s individual goals. You will also be charged with new client acquisition through sector engagement and networking across Sydney to support our business growth. You will be part of a dynamic working environment which will provide opportunity for ongoing training and support throughout your career.

To succeed in the role, you must have but not limited to:

  • Have suitable and relevant qualifications
  • Solid work experience in the Disability and/or Aged Care sector(s)
  • Have sound knowledge of the NDIS
  • Have previous care plan/health assessment experience
  • Be passionate about providing quality supports to individuals
  • Time management and organisational skills & ability to manage competing priorities
  • Well-developed ability to build and maintain strong relationships at senior levels of business
  • Relationship development skills to support diverse clients to achieve their goals and objectives
  • Current drivers’ licence and reliable vehicle
